WIAA Physical Form Card

A valid physical is required to be on file for participation in all sports offered through Mt. Horeb High School: Physicals April 1-December 31 are good for the current school year & the following 2 school years. Physicals January 1-March 31 are good for the the current school year & the following school year. Emergency Contact Information is completed via the annual online via Student Registration & no longer is a paper form.

 
Concussion and Sudden Cardiac Arrest Waiver

Required for athlete & parent/guardian to complete each year - waiver to complete SHOULD be done via the Annual Update online. Visit your Parent Portal; under the More section complete the Annual Update Registration - be sure to say "Yes" to the question asking if your student is participating in Athletics.

 
Student Transportation Agreement Co-Curricular Activities
Required for a parent to complete when transporting an athlete home from a game or event. Must be completed & turned into your coach on the day of the event. Please see form for more details.
