Code of Conduct

Every year, a parent/guardian & students involved in a co-curricular activity and/or sport are required to watch & listen to the Code of Conduct. At the conclusion of watching the annual Code of Conduct presentation, please complete the agreement via the Google Form.

2026-27 MHHS Code of Conduct - video & google form

Review the 2026-27 Code of Conduct, watch the video*, and then complete a Google Form with parent & student information. Once the Google Form has been submitted, you have completed the Code of Conduct requirement. 

*At the conclusion of the video, hit the "NEXT" button, completing all pages of the acknowledgement form. Please hit "SUBMIT" at the end of completing the form to be sent to the High School office.
